We invite you to join our team as a Registered Nurse Case Manager (RNCM) in our growing hospice agency. In this role, you will be at the heart of providing end-of-life care, focusing on enhancing the comfort and quality of life for patients and their loved ones during this sensitive time.
Responsibilities:
Take primary responsibility for a caseload of assigned patients, executing all phases of the nursing process.
Deliver skilled nursing care in accordance with established care plans, ensuring patient comfort and dignity.
Collaborate with attending physicians, interdisciplinary team members, and external agencies to coordinate optimal patient care and resource utilization.
Conduct thorough assessments, monitor patient conditions, and evaluate medication needs.
Work with interdisciplinary teams to create and implement individualized care plans tailored to each patient's unique situation.
Offer emotional support and guidance to patients and their families, ensuring they feel valued and heard.
Educate patients and caregivers about hospice philosophy, disease progression, symptom management, and end-of-life care options.
Maintain accurate patient records and documentation in line with agency policies and regulatory standards.
Participate actively in interdisciplinary team meetings, ensuring care plans reflect current patient needs.
Assess the need for caregiver and Certified Nursing Assistant (CNA) services and conduct supervisory visits as required.
Provide on-call coverage as scheduled and maintain annual clinical competencies.
Respect patient and staff confidentiality at all times, promoting a culture of trust and respect.
Participate in staff meetings, in-services, and committee events as needed.
Engage in personal and professional growth, adhering to state licensing and certification requirements.
Model a positive and collaborative team spirit, embodying the agency's hospice philosophy and values in all interactions.
Qualifications:
Bachelor's degree in nursing (BSN) required.
Active RN license in the state of Oregon.
A minimum of 2 years of nursing experience preferred, with previous exposure to hospice or palliative care.
Familiarity with Memory Care, Residential Care Facilities, Assisted Living, and Adult Family Homes is advantageous.
Strong communication and interpersonal skills, coupled with a compassionate attitude.
Valid driver's license, reliable transportation, and car insurance.
